This happened a few years after the story with the physician’s assistant. I had since moved and so I had a new doctor, and I was going for my annual physical. She was in her 40s, with blonde wavy hair to her shoulders, and a slim figure. She noted in my chart that I had a condition called Peyronie’s disease and she asked about it. I said it wasn’t too bad, and didn’t prevent me from having sex as long as I was careful when inserting because if I jarred it as I pushed in, it could bend upward and be painful for me. She asked if I had seen urology for it, and I said yes but I hadn’t undergone any treatment because the urologist at the time thought that the surgery was more risky than to just leave it alone. She asked if I had any other complications since then, and I told her that occasionally after sex I get a hardening of the lymph vessels that go around the right side under my glans and stays that was for a while, but I wasn’t sure if it was related to the Peyronie’s. Then I said that I used to have an issue where my glans would get pulled downwards when I was erect because the skin was a bit tight, but that I had a surgery to correct it. She said “a circumcision?” And I said, “oh, no, this is the little bridge of skin that connects the shaft to the glans…” She said “oh, you had your frenulum removed.” I said yes.
She asked if she could examine me, and told me to lower my pants and underwear while she swivelled around in her chair to get some gloves. As she turned back around, she rolled her chair forward so that she was at eye-level with my penis. Honestly it was a bit hot because she was an attractive woman and as I stood and she sat in front of me I could see her cleavage, which was average size with a nice plumpness to it.
As soon as she saw my penis she said “did you ask them to keep your foreskin when they removed your frenulum?” I said “no, it was just the frenulum that was tight.” She looked up at me and said “normally if you aren’t circumcised at birth they would have just done it when they removed the frenulum. It’s rare to leave it because sometimes it doesn’t heal as well, I’m surprised.” Then she asked where the lymph vessel hardening happens. I told her around the right side just beneath the glans. She pulled back my foreskin to have a look and asked if I was home birthed. I said “yeah I was, how did you know?” and she said “well most boys get circumcised before leaving the hospital, so it’s a pretty good guess. Sometimes the moms who home birth will ask me to do it when they come for their appointment. I’ve done it a few times but I always feel terrible.” I said “sounds like you don’t recommend it?” She said “There’s no strong recommendation. If you read the guidelines, there are some overall benefits to doing it, but most men will be fine not doing it. But in your case it looks like the lymph vessels that are hardening are very superficial, just running through your foreskin. In your case you might be better off if you did get a circumcision.” I said I didn’t know, I would be worried that sex wouldn’t feel as good for me or my wife after." She said “It’s hard to know, from what I’ve read there isn’t a big difference but because most men get it done as babies it’s hard to compare.” I said, “mmm, yeah, makes sense. But what about my wife, do you think it wouldn’t be as good, or I’d need to use more lube if I did that?” She said “I don’t think so. You could ask her what she thinks, but most women like it. I wouldn’t worry.” Then she asked if I wanted to make a urologist’s visit to look at it all and get their opinion." I said sure, (but then I never really followed up on that.) The she said “ok, you can pull your underwear back up,” and continued with the exam.
