I agree it's not 100% clear but at 7:05 you can see what look like little spots of scar tissue from the sutures, which prob means, as SRS wrote, that he was cut as an adult. This would also explain the looser cut and why the scar line looks and behaves like Rocco's.
It's even more obvious in some of the other scenes, esp if you select the highest viewing quality.
